Extended Access in our practice survey
Extended access is provided by a group of GP practices, (primary care network) over and above the core 8am-6pm contract. We are undertaking a patient survey, to ensure this meets the needs of our patients. The service is due to be provided from 1st October 2022.

Spring booster for over 75s and immunosuppressed
We will be holding vaccine clinics for the spring booster (ONLY for the over-75s and immunosuppressed) on Saturday 23 and Sunday 24 April 2022. If you are in these groups and received your last covid vaccine before 30 November 2021, please call us (01439 788215 option 4) to book an appointment.
Wearing of masks in GP surgeries/hospitals
Although some restrictions have been eased in the 'living with covid' plan, all patients, staff, and visitors are still required to wear masks (unless exempt) when attending the GP surgery, or when attending or visiting any health-care settings, including hospitals and care homes. Unless you are exempt, you must still wear a mask before coming into either of our surgeries.
CAMHS first contact practitioner
Ampleforth & Hovingham surgeries, together with our primary care network partners in Malton, Ayton & Snainton, and Sherburn, is looking for a full-time CAMHS first contact practitioner to work across our practices. The closing date is 6 March 2022.

Recording Covid vaccines received outside England
If you received any or all of your Covid vaccines outside of England (this includes Scotland, N. Ireland, and Wales, as well as overseas), and they are recognised in this country, you can now book a face-to-face appointment at some vaccination centres to have this recorded officially. Fundraising and Patient Donations
Thank you for all your kind donations and purchases from the book and DVD sale. We have so far raised a total of £889.76 from the sale of books and DVDs. We have also received a total of £1,372 from individual patient donations. Donations will be used to purchase medical equipment for the surgery.

What is the Yorkshire & Humber Care Record?
At the moment, every health and social care organisation that you use has a different set of patient or service user records for you. These records may duplicate information or one record might hold information about your treatment, care and support that another one doesn’t. To provide the best care to you as a patient or service user it is essential that health and social care professionals have access to the most up-to-date information.
It will be a secure virtual health and social care record. It will pull key information from different health and social care records and store it in one combined record. This enables health and social care professionals to find all the key information for your care in one place.
Why do you need to share my information?
To provide the best treatment, care and support to you as a patient or service user it is essential that health and social care professionals have access to the most up-to-date information. Information is already shared between health and social care organisations by phone and through paper records. Sharing health records allows this sharing process to happen more efficiently, enabling better care for you.
Healthcare organisations across Yorkshire and Humber are working together to further improve health and social care across the region. Health and social care professionals have a duty to share relevant information between them if they are involved in providing you with care.
Types of information that may be shared include:
- Address & telephone numbers
- Problem diagnosis list
- Medications prescribed
- Recorded allergies
- Care plans outpatient appointments, inpatient stays & clinical contacts
- Referrals, clinic letters & discharge information
- Test results
- Community treatment orders
- Contact details of care professionals
How do I know my records are kept secure?
By law, everyone working in, or on behalf of, the NHS and social care must follow strict information governance rules designed to respect your privacy and keep all information about you safe. Information is held on secure, encrypted systems which keeps a record of everyone who has accessed a patient record, the time and date when they accessed it and the information they were viewing. The laws on data protection are clear and we take them very seriously. We regularly check to make sure that only people who need to see your patient record are viewing it.
Can anybody see my records?
Your medical records will still be confidential. They will only be looked at by people who are directly involved in your care. Your information isn’t shared with anyone who doesn’t need it to provide treatment, care and support to you.
Your details will be kept safe and won’t be made public, passed on to a third party who is not directly involved in your care, used for advertising or sold.
Accessing your own health and care records?
Patients have the right under Section 7 of the Data Protection Act (1998) to request access to any information that an organisation holds about them. Each individual organisation that contributes information to your Yorkshire & Humber Care Record has a responsibility to handle these “Subject Access Requests”.
Should you wish to access your records this way, contact the organisation who holds the part of the record you are interested in directly, e.g. your GP, hospital, mental health trust or social care team.
Can I object to my records being shared?
Yes. You have the right to object to your health and care records being shared. However, if staff require access to the information shared on our Yorkshire & Humber Care Record to provide safe individual care they are legally allowed access.

What do I do if I would like to make a complaint about improper use?

Royal College of General Practitioners (RCGP) Research and Surveillance Centre (RSC)
This practice is one of over 260 practices in England contributing pseudonymised data for national research and surveillance.
These data enable continuous monitoring of infections and diseases in the community and is used in ethically approved research. The RCGP RSC is the main source of information for Public Health England (PHE) and helps with prediction and management of flu outbreaks and pandemics.
Providing pseudonymised data does not affect patients, their care or privacy, however if you no longer wish to allow your information to be used, please speak to your GP.
The pseudonymised data, extracted by an information service provider, Apollo Medical Solutions, is processed within the private and secure network of the Clinical Informatics and Health Outcomes Research Group at University of Surrey under a formal data sharing agreement. The pseudonymised data may be linked with other NHS data for analysis, including hospital episode statistics.
